Financial Policies
At Ashworth Chiropractic, we aim to provide clear and transparent information about your financial responsibilities to make your experience as stress-free as possible. Please review the following policies to understand how we manage payments and insurance:
Insurance Coverage
- Eligibility Verification: Patients are responsible for confirming active insurance coverage for their date(s) of service. If coverage is denied, the patient will be responsible for all charges.
- No Insurance Card: If you do not provide your current insurance ID card, we will still provide treatment. However, you will be responsible for all charges if your eligibility cannot be confirmed.
- Policy Limitations: Patients are responsible for any costs exceeding their policy’s visit limit or for non-covered services.
Note: Many of the services we offer are considered non-covered services by insurance companies. These include therapies, treatments, and exams that may not meet the insurance provider’s definition of “medical necessity.” For more details, please visit our dedicated page on Insurance Non-Covered Services.
Out-of-Pocket Expenses
- Co-Pays and Deductibles: Payment, including co-pays and deductibles, is required at the time of service. Any remaining balance will be charged to the card on file unless prior arrangements are made.
- Payment Plans: Flexible payment options are available. Please speak with our team for more information.
- ChiroHealthUSA: is a program designed to help patients save on out-of-pocket expenses for non-covered services. For more information or to enroll, visit ChiroHealthUSA.
Initial Exam and X-Rays
An initial exam and sometimes x-rays are required for all new patients to ensure you are a good candidate for chiropractic care. This exam allows the doctor to form a proper diagnosis, treatment plan and to justify care is appropriate. Some insurance policies may not cover the exam or charge different fees. Patients are responsible for any charges not covered by their insurance.
Financial Options
Self-Pay or Uninsured Patients
We offer discounts on services when payment is made by cash, check, or credit card at the time of service.
Commercial Insurance Patients/Medicaid
We verify benefits and eligibility when possible. However, quotes from your insurance provider are not a guarantee of payment. As a courtesy, we file claims for you, but patients are responsible for deductibles, co-pays, or non-covered services. Discounts are available for patients paying in full at the time of service for non-covered services via ChiroHealthUSA membership.
Medicare
Medicare only covers manual manipulation of the spine. All other services (e.g., exams, x-rays, therapies) are not covered and are the patient’s responsibility. Discounts are available for patients paying in full at the time of service for non-covered services via ChiroHealthUSA membership.
ChiroHealthUSA
We are proud to accept ChiroHealthUSA, a program designed to help patients save on out-of-pocket expenses for non-covered services. For more information or to enroll, visit ChiroHealthUSA.
Special Circumstances
Worker’s Compensation
Patients must inform their employer of their work-related injury and provide insurance carrier details. If this information or a settlement is not provided within three months, the patient is responsible for unpaid fees.
Personal Injury or Auto Accidents
Please provide your auto insurance card, health insurance card, attorney’s contact information, and a police report or accident description if possible. Claims will be filed in accordance with Iowa law: Med Pay first, then health insurance, followed by any third-party insurance.
Flexible Spending Accounts (FSA) / Health Savings Accounts (HSA)
We accept FSA and HSA plans. Statements for reimbursement are available upon request.
Payment Plans
We understand that unexpected healthcare costs can be difficult to manage. Flexible payment plans are available to ensure you receive the care you need when you need it. Please ask our team for details.
